What Is PACE?

Programs of All-Inclusive Care for the Elderly (PACE) is a comprehensive care model for adults 55 and older who need nursing home-level care but prefer to live in their homes or communities rather than in a nursing facility. PACE programs provide all Medicare and Medicaid (Medi-Cal in California) covered services — and then some — through a single coordinated interdisciplinary care team.

PACE is not a Medicare Advantage plan or a Medigap policy. It is a distinct care model that fully integrates Medicare and Medicaid (Medi-Cal in California) benefits into one coordinated program. It covers medical care, dental, vision, hearing, mental health services, physical and occupational therapy, prescription medications, nutritious meals, transportation to PACE centers, and social services — all through one team that knows your complete health picture.

Who Qualifies for PACE?

  • Age 55 or older
  • Live in a PACE organization's service area
  • Need nursing facility-level care as certified by the state
  • Be able to live safely in the community with the support PACE provides
  • Must qualify for Medicaid (Medi-Cal in California) in most cases, though private-pay PACE is available in some programs

Dual eligible beneficiaries: PACE is particularly well-suited for people who qualify for both Medicare and Medi-Cal. For those who qualify for both programs, there is typically no monthly premium and no copays for PACE services. The cost is covered by the combination of Medicare and Medi-Cal payments to the PACE organization.

What PACE Covers

Medical Services

Primary care, specialist visits, hospital care, emergency services, lab work and diagnostics — all coordinated through your PACE care team

Dental, Vision & Hearing

Comprehensive dental care, vision services, hearing evaluations and aids — services not typically covered by Original Medicare

Therapy Services

Physical therapy, occupational therapy, speech-language pathology — designed to maintain and improve your functional independence

Social Services

Social work services, caregiver support, help coordinating community resources and navigating healthcare systems

Meals & Nutrition

Nutritious meals at the PACE day center, and in some programs, home-delivered meals — supporting overall health and recovery

Transportation

Transportation to and from the PACE center for day program activities, medical appointments, and other services

The PACE Day Center

Most PACE programs operate an adult day health center where participants spend time during the week receiving care, engaging in activities, and connecting with other participants. This social component is an important part of the PACE model — addressing isolation, cognitive engagement, and quality of life alongside medical needs. Participants generally attend several days per week based on their care plan.

How PACE Compares to a Nursing Home

PACE is specifically designed for people who would otherwise qualify for nursing home placement but choose to remain in the community. Research has consistently shown that PACE participants experience fewer hospitalizations, better management of chronic conditions, and higher satisfaction scores compared to comparable nursing home residents. For many families, PACE provides the intensive care support needed to keep a loved one at home — safely.

PACE programs operate in specific geographic service areas. Not every community has a PACE program, and those that exist have limited enrollment capacity.
